There is a part of the healthcare system that most people never think about until they are stuck in it. A doctor decides you need to see a specialist. They write a referral. And then, for many patients, nothing happens for days or weeks, if at all.
The numbers are startling. Only about half of all specialist referrals are ever completed. Securing an appointment takes an average of three weeks. And the reason is almost never a shortage of doctors. It is paper. Specifically, faxed paper.
At least 70% of healthcare providers in the US still exchange patient information by fax. That is not an old statistic. That is how a referral from your doctor reaches a cardiologist's office today, in 2025. A small administrative team on the receiving end, often the same two or three people who have worked there for decades, then manually reads that fax, pulls out the relevant details, and tries to call the patient. When the volume is high enough, patients simply fall off the list.
A practice losing 30% of its monthly referrals is losing tens of thousands of dollars in revenue every single month. But more importantly, a patient referred to cardiology who never gets scheduled is a patient at higher risk of a serious cardiac event. The administrative breakdown is a medical one too.
Basata, a Phoenix-based startup, just raised $21 million to automate this process specifically for specialty practices. Their system reads the incoming fax, extracts the clinical information, and calls the patient automatically. Practices can also route incoming patient calls to an AI agent that handles scheduling and routine tasks around the clock. Basata says it has processed referrals for roughly 500,000 patients, with 100,000 of those coming in a single month, which suggests real traction.
What makes Basata's approach worth watching is not just the technology but the strategy. Instead of trying to serve every medical specialty at once, they started with cardiology, then added urology. They reportedly turned down a large deal in a specialty they had not yet studied thoroughly enough. That kind of discipline is rare in the startup world, and it matters in healthcare where one wrong scheduling logic can mean a patient ends up with the wrong provider entirely.
But Basata is not alone. Tennr, founded in New York in 2021, has raised over $160 million and is now valued at $605 million. It trained its own AI model on more than 100 million medical documents to read and route referrals, including handwritten doctor's notes. Assort Health, another competitor, raised $102 million in just four months and has cut patient hold times at some practices from 11 minutes down to one minute. The total US market for this type of software was worth $7 billion last year and is growing at roughly 17% annually.
That level of investment, concentrated in one narrow corner of one industry, tells you something. The problem is large, the status quo is deeply painful, and the people who have to live with it every day are ready to change it. Administrative staff turnover in healthcare exceeds 25% annually in many organizations, largely because the job consists of repetitive, high-volume tasks with no end in sight.
The honest question underneath all of this is about jobs. These companies are automating work that humans currently do. Their current answer is that they are not replacing people, they are giving overwhelmed staff the capacity to handle more patients without burning out. That may be true today. But as these systems improve and practices see the cost savings, the pressure to reduce headcount will grow. It is the same dynamic playing out in back offices across every industry right now, and healthcare will not be exempt.
